Recreational scuba diving and free diving requires good physical and mental health. There are a few medical conditions which can be hazardous while diving. Those who have, or are predisposed to any of these conditions, should be evaluated by a physician. The Diver Medical Participant Questionnaire provides a basis to determine if you should seek out that evaluation. If you have any concerns about your diving fitness not represented on theform, consult with your physician before diving. If you are feeling ill, avoid diving. If you think you may have a contagious disease, protect yourself and others by NOT participating in dive training and/or dive activities. For your safety, and that of others who may dive with you, please answer ALL questions honestly!
PLEASE NOTE: All Non-Certified Divers and Students of Certification Courses with Dive Kauai will be REQUIRED to complete it electronically on your booking confirmation. Should you feel you may need medical clearance for any of the questions on the form, please PRINT the form and submit to your physician to complete PRIOR to your dive.
Dive Kauai offers scuba diving tours for non-certified divers, certified divers, and diver certification courses. First-time (non-certified) divers will experience the PADI Discover Scuba Diving Program, which covers the fundamental rules and guidelines of safe scuba diving.
